About Us -

2009 marks the 30th Anniversary of Haven Hospice, celebrating a 30-year tradition of excellence and innovation. Serving North Florida since 1979 and licensed as a not-for-profit hospice since 1980, Haven Hospice has been privileged to care for more than 45,000 patients and families. The organization is now home to seven offices, including four inpatient hospice care centers, covering 18 counties and more than 13,000 square miles. In 2008, Haven Hospice expanded its services to patients and families by opening new offices in St. Augustine and Deland, providing comprehensive care that includes programs and services many other hospices do not or cannot offer.

We honor and respect the cultural differences in the patients and families we serve and are dedicated to improving community access to hospice and palliative care. In addition to expanding care to those who need it, Haven Hospice is nationally recognized for excellence and innovation in the care we provide.

We are also recognized for our expertise in providing care to rural communities. The Center for Medicare and Medicaid Services selected Haven Hospice to participate in a project that studies access to hospice care for rural patients without caregivers. This demonstration project was awarded to only two hospices in the nation. Haven Hospice also pioneered freestanding hospice care centers throughout the North Florida region. Our four care centers are conveniently located to serve patients and families throughout our service area.

The Haven Hospice team is specially trained in managing symptoms, controlling pain, and caring for the psychological, social, emotional and spiritual needs of individuals and their loved ones. The team works with the individual, family and the patient’s primary physician to establish the plan of care.
