Position Summary:
Provides and Physical Therapy services to patients including assessment, treatment, program planning and implementation, related documentation and communication. Functions under physicians' orders.

Adheres to applicable principles and practices of Physical Therapy, Innovative Senior Care policies and procedures and state regulations.
Supervises Outpatient Therapy, home health & Wellness Programs

Recommended Minimum Position Qualifications:
1. Bachelor of Science, Masters, or Dr in Physical Therapy
2. WI state licensure for Physical Therapy

License to practice physical therapy within the state of Wisconsin Benefits: 401(k)
