The Clare Bridge Program Coordinator administers dementia programs to meet the specific needs of each and all residents within a memory care facility.

Key responsibilities include:

*Ensuring residents' lives are maintained to the extent possible by providing activities and contact with the community
*Ensuring residents are treated with respect and dignity, recognizing individual needs and encouraging independence
*Coordinating training and education of resident care staff regarding memory care program/activities on an ongoing basis - including classroom in-services and situation specific training.
*Coordinating and participating in special events at the community including family socials and open houses
*Developing and maintaining contact with community agencies and organizations
*Reporting changes in residents’ overall health to the nurse, including changes in behavior; taking actions to address concerns in a timely manner

We seek the following qualifications:

*Must have a Bachelor's degree (B.A.) in therapeutic recreation, Sociology, or activities-related field
*Must have a minimum of three or more years of direct programming experience with older adults and persons with dementia
*Must be able to evaluate resident's needs, supervising other employees, and in training volunteers
*Must have the ability to interact and build relationships with older adults
*Must have strong creativity and organizational skills
*Must have a clean driving record (verified by MVR) and willingness to drive company vehicle to transport residents
*Must have strong team building and mentoring skills
