    MINIMUM QUALIFICATIONS
    Education: Successful completion of State Approved Nursing Assistant Training and Competency Evaluation Program.
    EXPERIENCE
    Previous experience preferred.

    SPECIFIC REQUIREMENTS
    Must be able to read, write, speak, and understand the English language. Must possess the ability to make independent decisions when circumstances warrant such action. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public. Must possess the ability and willingness to work harmoniously with professional and non-professional personnel. Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents. Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices. Must be able to relate information concerning a resident's condition. Must not pose a direct threat to the health and safety of other individuals in the workplace.

    PURPOSE OF YOUR JOB POSITION
    The primary purpose of your job position is to provide your residents with routine daily nursing care in accordance with our established nursing care procedures, and as may be directed by your supervisors.

    DELEGATION OF AUTHORITY
    As a Certified Nursing Assistant you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

    JOB FUNCTIONS

    POSITION SUMMARY
    Assist professional nursing personnel by performing various resident care activities and related services necessary in caring for the personal needs and comforts of residents.

    WORKING CONDITIONS
    Works throughout the nursing service area (i.e., nurses' stations, resident rooms, etc.).
    Sits, stands, bends, lifts and moves intermittently during working hours.
    Is subject to frequent interruptions.
    Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
    Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.
    Communicates with nursing personnel and other department personnel.
    Works beyond normal working hours, on weekends and holidays, and in other positions temporarily, when necessary.
    Attends and participates in continuing educational programs.
    Is subject to falls, burns from equipment, odors, etc., throughout the work day.
    Is subject to exposure to infectious waste, diseases, conditions, etc.

    RESIDENT’S RIGHTS
    Maintain the confidentiality of all resident care information.
    Ensure that you treat all residents fairly, and with kindness, dignity, and respect.
    Ensure that all nursing care is provided in privacy.
    Knock before entering the resident's room.
    Report all grievances and complaints made by the resident to the Nurse Supervisor/Charge Nurse.
    Report all allegations of resident abuse and/or misappropriation of resident property.
    Honor the resident's refusal of treatment request. Ensure that such requests are in accordance with the facility's policies governing advance directives.

    PHYSICAL AND SENSORY REQUIREMENTS
    (With or Without the Aid of Mechanical Devices): Must be able to move intermittently throughout the work day. Must be able to speak and write the English language in an understandable manner. Must be able to cope with the mental and emotional stress of the position. Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met. Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies. Must be in good general health and demonstrate emotional stability. Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility. Must be able to lift, push, pull, and move a minimum of 50 pounds. Must be able to assist in the evacuation of residents during emergency situations.

    DEPARTMENT SPECIFIC REQUIREMENTS
    First Aid and CPR must be taken periodically
    Attends and participates in scheduled training and education classes
    Attends and participates in scheduled orientation program and activities
    Attends and participates in annual mandated OSHA and CDC in-services for hazardous conditions, TB management, blood borne pathogens.

    ADMISSIONS, TRANSFER AND DISCHARGE FUNCTIONS
    Admission, Transfer and Discharge Functions
    Ensures that resident’s room is ready for receiving the resident
    Greets resident, escorts them to their room and makes them comfortable in their new surroundings
    Introduces resident to other residents and staff as appropriate
    Assists resident with packing their personal possessions when they are moving to a new apartment or being discharge
    Assists resident with loading/unloading residents from vehicles as necessary

    DELIVERY OF CARE (MAINTAIN CONFIDENTIALITY)
    Answer pages promptly and identifies emergency situations and responds with appropriate measure
    Receives assignments from Supervisor to provide care for residents.
    Checks resident in neighborhood regularly and follow through with problems or needs by delivering care or reporting to Supervisor
    Maintains knowledge of resident’s condition, notes any significant changes and communicates this information to appropriate staff.
    Respects privacy, knocks and waits for acknowledgement before entering, provides privacy and explains procedures of resident before administering
    Identifies safety and security hazards and reports back resident and environmental needs
    Follows established infection control policies, procedures and precautions, adheres to Universal blood and body fluids precautions
    If resident is on I&O empties urinary drainage bags and records amounts in record and reports irregularities to nurse
    Uses daily assignments sheets
    Assists resident with personal care, fingernails, hair, oral hygiene, deodorant if directed, skin moisturizers if directed and dressing following precautions and encourages resident to do as much as he or she can do for him/herself
    Assists with resident’s physical needs including eating, toileting, assistance following incontinence whole encouraging them to do as much as possible for themselves
    Follows policies and procedures relating to body mechanics, transfers, body positioning and mobility
    Cares for resident’s prosthesis as required
    Assists with transferring resident from chair to bed and back, in and out of wheelchairs and with assistive devices
    Assists with medication reminders, hydration and monitors nutrition
    Delivers meal trays to residents who are unable to go to the dining room
    Provides in house escort services to beauty shop, activities, meals guidance and assistance with phones
    Leads, organizes, participates in and assists with resident’s activities
    Performs specific work duties and responsibilities as assigned including but not limited to daily tidying of rooms, assisting and completing resident personal and flat laundry, laundering dining room linens, setting and clearing tables

    RESIDENT SUPPORT
    Welcomes and shows empathy to new resident and helps them adjust to new environment
    Assists resident, family and staff to ensure a comfortable homelike environment, I.e. assists resident to personalize room, monitor light , sound levels, and temperature
    Establishes and maintains good public relations with residents, families, and other guests
    Reports all complaints and grievances made by the resident

    SAFETY AND SANITATION
    Handwashing before and after any service to the resident
    Reports immediately to the supervisor
    Resident leaving or missing from facility
    Hazardous conditions and equipment
    Communicable or infectious disease

    SAFETY RULES AND PRECAUTIONS
    Follows policies and precautions in performing duties
    Performs routine housekeeping duties, keeps floors dry, reports spills
    Cleans, disinfects and returns all resident car equipment to storage after each use
    Before breaks and end of day, stores all tools, equipment and supplies to proper place
    Follows established isolation precautions and procedures and use of disposal
    personal protective equipment
    Smoking: follows established regulations and reports violations
    Assists with maintaining cleanliness of resident apartment and work areas and if instructed straightens dresser tops and stands.
    Information Reporting (to Nurse Supervisor)
    Utilizes designated reporting tools as required and participates in reporting Quality Indicator Information
    Reports significant changes in condition, appetite or hydration of resident
    Reports incidents and accidents
    Reports family inquiries

    SUPPLIES AND LINENS
    Monitors resident supply inventory daily and charts appropriate charges
    Utilizes supplies and equipment properly without waste
    Changes and makes beds
    Collects, washes, folds soiled linen and personal clothing in assigned days./shifts as per personal care plan

    FOOD SERVICES
    Escorts resident to dining room
    Prepares resident for meals, bathroom,wash hands, comb hair, position tables,clothing protector
    Checks menu’s for name and diet ordered before serving
    Assists resident if requires help with eating process and identifying food arrangements
    Serves and collects dishes and charts meals was accepted on meal log and offers alternative approved substitution for food not eaten
    Reports changes in eating habits

    PERSONNEL FUNCTIONS
    Performs all assigned asks in accordance with established policies and procedures and as instructed by Supervisor
    Follows work assignments, and/or work schedules in completing and performing assigned tasks
    Cooperates with inter-departmental and facility staff t ensure that nursing services meet the needs of our residents.
    Meets regularly with shift nurse staff to identify and assist in correcting problems areas or improvement of services
    Executes all terms and conditions in Employee Handbook
    Notify facility when you will be late to work or absent. Maintains a dependable attendance record
    Performs any miscellaneous work assignments as may be required
    Thinks and acts calmly and logically to meet unusual occurrences of he job without being thrown off stride
    Adheres to standards of cleanliness, grooming, hygiene, and dress code.
    Prevention and control of infection (before care)
    Major incidents and emergency procedures (within 30 days of employment)
    Residents Rights and Abuse and Neglect (within 30 days of employment)
    Residents Needs and ADL’s (within 30 days of employment)

    HIV/AIDS:2 hr in-service within 6 months of hire (1 hr every 2 years)
    Alzheimer’s Level I (within 3 months of hire)

    ECC In– Service ( within 6 months of hire)
    Limited Mental Health (within 6 mos. LMH license or of hire
