Clinical Supervisor - RN

ABOUT AMERICAN ELDERCARE, INC.

STATEMENT OF RESPONSIBILITY:

  • Provides direction and supervision to clinical staff on individual patient plans of care;
  • Provides case management to assigned caseload and facilitates care with consistency to the Regulations and Agency policy.


JOB KNOWLEDGE:

  • Knowledge of home health nursing techniques, including: patient assessment, care planning, nursing procedures, patient teaching, and evaluation.
  • Skilled with interpersonal communications.


OTHER:

  • Maintains current CPR certification.
  • Able to work with limited supervision.
  • Good organizational skills.


FUNCTIONS:

  • Obtains/reviews referral information to determine appropriateness and assigns direct care staff according to patient needs.
  • Obtains physician orders for certification and recertification of home health services; Communicates order changes to appropriate staff in timely manner.
  • Provides case management for assigned patients through close communication and coordination with direct care staff, family, physician and others.
  • Works with nursing staff to evaluate and update patient care plans and goals; assists staff toward patient outcomes, as appropriate.
  • Makes direct in-home supervisory visits, as needed.
  • Serves as resource to nursing and other direct care staff on patient care issues, directs staff in care techniques, as appropriate.
  • Assists with orientation and performance evaluation / competency review of all direct care staff. Works with Director of Patient Services to improve staff skills.
  • Provides regular and periodic reports to Director and/or Administrator on patient care issues.
  • Participates with management team in establishing department goals.
  • Provides direct patient care, as needed, to assist with patient caseload needs and/or to complete admission requirements in a timely manner.
  • Assists with performance improvement initiatives, as requested, to improve overall agency quality of care.
  • Maintains infection control practices; consistent application of infection control and safety policies.

 
REQUIREMENTS:

  • Registered Nurse, Licensed in the State of Florida
  • Demonstrated expertise in home health and supervision
