Care Manager Assistant/ Case Manager Assistant



ABOUT AMERICAN ELDERCARE:

POSITION:
We are seeking an organized and compassionate individual to provide assistance and support to the Care Managers in provision, planning, coordinating, and managing of care plans developed for each member in the Long-Term Diversion Program. This individual is also responsible for accurate record keeping, assisting in the coordination of services and following policies and procedures set by the Plan.


QUALIFICATIONS:

  • Must have experience in the social services field.
  • Possess knowledge of geriatric issues and is sensitive to this populations needs.
  • Must possess excellent verbal and written communication skills.
  • Organizational skills are required.
  • Must a valid driving license and own a licensed and insured vehicle.



FUNCTIONS:

  • The care manager assistant is responsible for providing support services to care managers as requested by a Senior Care Manager or Care Manager Supervisor.
  • Assist in the coordination of services with network providers and ensure requested services are provided in a timely manner.
  • Assist in the coordination of informal services.
  • Conduct monthly ALF visits and document all interventions, interactions and observations in the members file as requested.
  • Provide ongoing support to members of the plan and their caregivers.
  • Assist in application for Medicaid as requested by Care Manager Supervisor.
  • Develop and maintain a positive working relationship with Providers and other AEC departments (i.e. Home Health, Accounting).
  • Assist in the resolution of eligibility issues.
  • Track Medicaid appointments to ensure members do not lose eligibility.
  • Assist in making monthly calls to home members and documenting those calls.
  • Help prepare and mail “Welcome Letters” and “Physician Care Plan Letters.”
  • Implement all policies and procedures.
  • Performs other duties requested by management
  • Some travel required.
