Community Health Director - The Gardens at Town Square


Era Living, a respected and recognized leader in retirement living throughout the Puget Sound, has an outstanding opportunity for a Community Health Director at The Gardens at Town Square.

The Gardens at Town Square is located in the heart of downtown Bellevue, offering luxurious apartment living for adults who've reached the age of 62. The Gardens at Town Square offer a superior level of comfort, quality and the personalized hospitality for which Era Living communities are known

The Position
The Community Health Director is responsible for managing Health and Wellness services and programs for the community. This includes the supervision of staff, resident case management, overseeing the Wellness Clinic, health education and promotion activities. Primary responsibilities include:

  • Direct and indirect supervision of the staff providing services to the residents in the Boarding Home licensed apartments.
  • Indirect oversight of residents living in non-licensed apartments as well as the direct scheduling of skilled and non skilled service professionals in the Wellness Center.
  • Providing community's resident case management, health education and health promotion activities that focus on the needs of older adults.
  • Supervising, directing, and evaluating all direct care services provided in the Wellness Center.
  • Designing Wellness Plan for all residents.
  • Ensuring coordination of sub-contracted health services (dentist, podiatry, audiology).
  • Providing educational programs for residents through the whole community.

    The Successful Candidate
    The successful candidate for this position will have five (5) years experience as an RN and three (3) years experience in settings working with older adults, Assisted Living or Skilled Nursing Facilities.

    Qualifications include:
  • Current WA State RN license
  • Minimum 5 years RN experience
  • Expanded knowledge of WA state Boarding Home regulations
  • Extensive knowledge of residential and long-term care, gerontology and care of older adults
  • Experience and skill in managing persons with cognitive impairment
  • Proven success in teaching and training paraprofessionals concerning senior care issues
  • Mental Health and Dementia Specialty training and Modified Fundamentals of Caregiving preferred (will need to obtain if hired)
  • Fantastic customer service skills
