| Job Description: |
- Licensure Required - 1-2 years experience is required Full time RN who coordinates the Bridge Program including all aspects of identifying and moving hospice appropriate VNA patients to MVH. Responsibilities include c ollaborating with VNA managers on a regular basis to review cases for hospice appropriateness and to identify strategies for admitting to MVH and w orking with VNA nurses to admit hospice appropriate patients to MVH.
Qualifications: At least two years experience as an RN and e xperience with program development and implementation.
