| Job Description: |
Responsible for coordinating all admissions into the Care Center, including internal transfers. Completes Medicare documentation and other admission documentation and facilitating communication of admission to Care Center departments. Acts as the liaison between external referral sources for the care center. SKILLS & EXPERIENCE: 1-3 Years admissions experience in a Skilled Nursing Facility Working knowledge of Federal, State and local privacy and confidentiality requirements Knowledge of Medicare requirements Proficient with Microsoft Office programs Strong oral and written communication skills High School Diploma or GED Equivalent Strong organizational skills EEO M/F/V/D |
