The Registered Nurse provides skilled nursing care to SDH terminally ill home care patients in accordance with the treatment and plan of care dictated by patient, family, physician, and members of the interdisciplinary team. The RN works in collaboration with the other disciplines on the team to coordinate care for the patient. Employee is expected to learn and utilize a windows compliant computer system. Proficiency and appropriate utilization is a requirement of the job role.
Education: Must be a graduate of an approved school of professional nursing, who is currently licensed as a Registered Nurse by the State of California. A Bachelor's or Associate's degree in nursing from a program accredited by the National League of Nursing is highly preferred. A Public Health Nurse credential is desirable. Must possess a valid CPR certification. Bilingual abilities preferred.
Experience: Requires a minimum of a) one year acute care or home health experience as a professional nurse within the last 3 years or b) a baccalaureate degree in nursing from a program accredited by the National League of Nursing. Good knowledge of home health care principles and practices within the scope of palliative medicine. Familiarity with interpersonal dynamics operative within a health care team and knowledge of the dying and bereavement processes. Possess basic knowledge or aptitude to operate a computer, knowledge of basic software programs is preferred. Current California Driver's license and automobile insurance required.
