Job Description: 

AccentCare , a national home healthcare leader, is now hiring for a Clinical Supervisor in our Rancho Cordova office in Sacramento, CA.   

 

POSITION SUMMARY:

Responsible for the supervision of clinical staff, the maintenance of quality client care in compliance with federal, state and local regulations and accreditation standards and adherence to budget goals.

 

ESSENTIAL FUNCTIONS:

  • Responsible for the supervision of clinical field staff.
  • Support field personnel in decision-making with regard to patient care, including setting of short- and long-term goals (possibly among multiple disciplines), as needed.
  • Timely review and sign-off on admissions paperwork, physician orders, supplemental physician orders, transfers and discharges and all patient charts in relation to proper diagnosis and coding.
  • Review patient information in the clinical record to evaluate the level of care and adequacy of services delivered and engaging clinical field staff to make any necessary revisions to patient charts.
  • Manage visits per episode to ensure proper and complete case management in order to reduce low utilization payment adjustments (LUPA).
  • May be required to perform nursing functions for patients or act as case manager.
  • Review physician orders including referrals.
  • Attend/conduct clinical record review per agency policy.
  • Conduct orientation, competency testing and field observation of personnel in coordination with the Director Patient Care Services (DPCS), as directed.
  • Evaluate systems, patient care, and adequacy of staff for patient census.
  • Participate in Performance Improvement activities, in-service education training, case conference, and staff meetings, as required.
  • Evaluate agency field staff through supervisory visits, performance review process, and on-going support, as required, by the agency.
  • Remain available on the premises or accessible by telephone or email at all times during hours of operation.
  • Ensure timely and accurate entry of paperwork into the MiSys system.
  • Act as liaison between clinical staff and physician when clarification is needed in regards to patient care.
  • Coordinate patient care among various disciplines (ex: PT, OT, SLP).
  • Direct the calendar of field clinicians and ensure that schedules are maintained.

    REQUIREMENTS:

    • Current valid RN license, BSN/PHN preferred
    • Current CPR/BLS certification
    • Two (2) years of management experience as a Registered Nurse within the last five (5) years in a home health agency, primary care clinic, or health facility preferred
    • Must have own means of transportation, valid CA driver’s license, proof of auto insurance and registration
    • Must have the ability to articulate, read, and write and follow instructions in English, as well as, effectively communicate in both written and verbal form
    • Must have excellent clinical judgment, observation and communication skills and demonstrate the ability top coordinate care of multiple patients
    • Must be computer literate in Microsoft Office software (Word, Excel, Outlook)
