Job Description: 
  • The Access Center RN provides skilled nursing care to SDHIPM patients in accordance with the treatment and plan of care directed by patient, family, physician, and members of the interdisciplinary team. May also provide work direction to other team members. Care is provided directly and within the interdisciplinary team. The Access Center RN will provide staffing support to Access Center patients. May also assist with other HC patients, Crisis Care, Admissions, GIP patients as needed. Employee is expected to learn and utilize a windows compliant computer system and Misys documentation system. Proficiency and appropriate utilization is a requirement of the job.

     

    Education: Must be a graduate of an approved school of professional nursing, who is currently licensed as a Registered Nurse by the State of California.   A Bachelor’s degree in nursing from a program accredited by the National League of Nursing is preferred.   A Public Health Nurse credential is desirable.   Must possess a valid CPR certification.   Bilingual abilities preferred.

    Experience: Requires a minimum of one year acute care or home health experience as a professional nurse.   Good knowledge of home health care principles and practices within the scope of palliative medicine.   Familiarity with interpersonal dynamics operative within a health care team and knowledge of the dying and bereavement processes.   Possess knowledge or aptitude to operate a computer, knowledge of medical charting software is preferred. Must possess excellent assessment, excellent oral and written communication skills, superb organizational skills and ability to prioritize multiple tasks.    Current California Driver’s license and automobile insurance required.

    • Must be thorough and detail oriented
    • Use ethical principles in decision making and practice
    • Comfortable in maintaining professional boundaries
