POSITION SUMMARY

The On-Call Hospice nurse is a licensed R.N. who is responsible for providing patient care and the delivery of quality comfort care to patients and their families, after regualr work hours, on weekends and holidays.   May be assigned admissions or RN triage duties.   Member of IDT (Interdisciplinary Team)

ESSENTIAL FUNCTIONS

  • Assures all services indicated in the Plan of Care are provided.
  • Develops a detailed Plan of Care for Each assigned patient in conjunction with IDT, patient and family.
  • Supervises Hospice Aides.
  • Prepares clinical notes that demonstrate progress toward established goals.
  • Reports to the attending physician, hospice medical director, and core team members any significant changes in the patients' condition and requests change of orders as needed.
  • Responsible for pain and sympton control.
  • Reviews with patient and family services that are available.
  • May rotate on-call duties as necessary.
  • Completes all required documentaton accurately within 72 hours.
  • Participate in in-service programs.
  • Performs other duties as assigned.
  • Participate in the QAPI hospice performance improvement program.
Required Skills

Identifies who are approaching last days of living.   Communicates honestly to patient/family about approaching death and gifts of last days.  Speaks of death as natural process not failure of treatment.   Determines patient/family/wishes regarding place of death and seeks to have death occur where desired.   Assist family to give patient permission to die, to "good bye" and to bring reconciliation to family relationships.

 

Ability to demonstrate conduct confonning to a set of values and accepts standards.

Required Experience

Education:  Minimum, eduction leading to R.N., Bachelor's degree preferred

Experience:  1-3 Years of Experience in hospice, home care, long term care preferred

Computer Skills

To perform this job successfully, an individual should have knowledge of Internet software; Word Processing software, Healthcare software.

Certificates & Licenses

Current Registered Nurse (RN) license in state where work is performed with no restrictions
