Area Vice President of Performance Improvement:

  • Develops and implements the locations quality improvement plan in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards.
  • Works with Divisional/Regional Leadership to develop and implement systems, policies, and procedures for the identification, collection, and analysis of performance measurement data. Position is clinically focused to improve and ensure quality care is being provided to our patients.
  • Design and conduct surveys to assess the achievements of Gentiva Home Health and provide feedback to senior leadership and staff.
  • Implement systems to ensure that services conform to regulatory requirements, contractual obligations, and Gentiva HH policy.
  • Educates and trains the leadership, staff and business associates as to the quality improvement plan and their respective responsibilities in carrying out the quality improvement program.
  • Leads, facilitates, and advises internal quality improvement teams.
  • Partners with internal audit and compliance to assist with quality control audits ensuring that Gentiva HH operations is prepared for the audits and that processes and documentation are compliant with required standards and, implement follow up actions.
  • Carry out credential processes to meet peer review requirements as part of Quality Improvement program and maintain updated provider enrollment information for health plans and insurance plans as applicable.
  • Collects and summarizes performance data, identifies opportunities for improvement, and presents findings quarterly to the Performance Improvement Committee and Board of Directors.
  • Analyzes customer survey data to identify opportunities for improvement and presents findings to appropriate departments.
  • Ongoing analysis of data to identify trends and areas in need of improvement.
  • Develops plans of correction with collaboration of the Director of Clinical Management and the ASDS (education and training department) in the development of training plans to correct issues identified.
  • Performs Chart Audits and is responsible for documentation improvement.
  • Responsible to review charts with respect to adverse events and Medicare Outcomes.
  • Performs QA SOC OASIS assessments and works with clinicians to improve their accuracy.
  • Participate in annual agency evaluations as may be applicable.
  • Attend Staff and Team meetings and Board meetings when requested/applicable.
Qualifications
  • Masters degree preferred in a relevant field (e.g., health information management, or other health care field/MPH) or equivalent.

  • Required a Bachelors Degree in clinical or allied health field or the equivalent plus a minimum of eight years clinical experience and three years experience in health information, quality utilization or risk management required.
  • Ability to prioritize quality improvement strategies and choose how best to narrow 'quality gaps' for clinical programs and outcomes.
  • Proven ability to achieve results.
  • Possess knowledge of the CQI process and the ability to apply to varied activities.
  • Ability to examine quality versus cost of programs/services.
  • Ability to communicate clearly, both verbally and in writing, with a diverse group of staff, both internal and external to the organization.
  • Excellent training, mentoring and interpersonal skills.
  • Proposal writing and analytical reasoning, ability to interpret and evaluate complex information.
  • Team development and facilitation techniques to help project teams achieve their highest potential.
  • Must have at least five years of experience in a supervisory role with at least three of years at senior level.
  • Certification as a RHIA or RHIT and Certification as a Professional in Healthcare Quality (CPHQ) also preferred.
  • Clinical licensure in state(s) of practice required.
  • Knowledge of statistics, data collection, analysis, and data presentation.
  • Knowledge of federal and state laws and regulations and accreditation standards.
