Heartland Hospice has an outstanding per diem opportunity for a qualified Professional to care for Hospice clients in their homes. This position includes a positive work environment, competitive compensation and mileage reimbursement. Must be fully licensed in state of Minnesota. Master degree required. 1 year clinical experience. Hospice experience strongly preferred. 100% travel necessary on a daily basis. Experience in varied multidisciplinary settings in the health care field. Experience working with patients and families coping with terminal illness and death. Excellent communication and interpersonal skills.

Master's Degree in Social Work. May require licensure or certificate as required by the state. Must be in good standing in the state of practice.
