Position Description

PACE (Program of All-inclusive Care for the Elderly) is a Medicare program for people age 55 or older that provides community-based care and services to people certified by the state as meeting the need for nursing home level of care. The care is provided by an interdisciplinary team of professionals who work with the patient and family to develop a plan of care. The PACE model was designed to provide the following benefits to the frail elderly:
• To stay in familiar surroundings (home vs. institutional setting)
• To maintain autonomy
• To maintain a maximum level of physical, social, and cognitive function

Under the supervision of the PACE Enrollment Director, obtains initial information relating to admissions, coordinates intake process, and provides information and referrals to community agencies as appropriate. Promotes the PACE Program to potential participants and community agencies.

QUALIFICATIONS, SKILLS AND ABILITIES

1. Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

2. Successfully complete a Competency Assessment upon hire and receive annual competency assessments

3. Must have a valid driver’s license, proof of insurance and have means of transportation.

4. Skills and Knowledge:
• Working knowledge of physical, mental and social needs of frail older adults.
• Effective skills in assessment of nutritional needs of frail older adults.
• Effective written and oral communication skills.
• Ability to work with an Interdisciplinary Team, as well as independently.
• Effective organizational skills.
• Basic computer skills.

SUPERVISORY RESPONSIBILITIES None.

EDUCATION and/or EXPERIENCE

1. Education: A Bachelor's Degree in business, or a health or human services field, or a high school diploma and office assistant training (preferably health care specific)

2. Experience: Two years of experience in a marketing and/or admissions role for a health care provider, or in a information and referral resource role within the senior network of Pinellas County. Shall have either training or related experience in the job assigned.


PHYSICAL DEMANDS

1. Physical Requirements – Requires standing, walking, pushing, pulling and lifting. Requires the ability to lift/carry up to fifty (50) pounds and push or pull objects exceeding one hundred fifty (150) pounds, using appropriate body mechanics. Requires manual and finger dexterity and eye-hand coordination.

2. Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Must be able to communicate verbally with all staff, caregivers, participants, and community at large.

3. Pressure Factor - Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled appointments. Subject to people that may have the potential for verbal or physical aggression.
