As part of the Interdisciplinary Group (IDG), the Admissions RN conducts the initial admission process for patients referred for hospice services. S/he initiates the plan of care based on identified immediate needs, and communicates with the IDG. Qualifications: • Current Florida license as RN, or current licensure from another state plus a temporary Florida permit. • Valid driver’s license and automobile insurance in the State of Florida. • Reliable transportation to meet visit schedule. • Minimum one (2) year’s RN experience where physical assessment skills were utilized • Ability to use equipment with visual and auditory mechanisms. • Ability to effectively communicate in English. • Flexible schedule to meet the needs of hospice patients. • Ability to physically access any home i.e. navigate stairs and narrow spaces, tolerate heat and lack of air conditioning, in any area Company serves. • Ability to manage the emotional stress of working with and caring for terminally ill patients and their families. • Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation. • RN, Preceptor: one (1) year's experience as an Admissions RN preferred, with demonstration competence in assessment and in depth understanding of Company systems. Job Responsibilities: Clinical • Assess patient/family and identify immediate needs including assessing psychosocial, spiritual care, and volunteer, and make appropriate referrals, as needed. • Initiate the plan of care that accurately reflects the patient’s immediate needs. • Evaluate patient/family for hospice admission, utilizing approved criteria and procedures. • Demonstrate clinical skills regarding: application of company policy and procedure, physical assessment, symptom management, and patient/family relationships. • Consult, refer, and collaborate with other professionals to promote quality and continuity of patient/family care. • Take verbal orders from physicians, ARNPs and physician’s assistants. • Educate patient, family, caregivers and other health professionals about disease process and decline, palliative interventions, care giving, dying process and safety practices. • Provide information regarding available community resources and other referral sources when hospice care may not be appropriate. • Provide timely and consistent feedback to referral sources, physicians, and patient/families. • Maintain regular contact with social workers, ALFs, nursing homes and other referral sources to facilitate the admission process. • Accept responsibility and is accountable for nursing actions: demonstrate an understanding of the legal consequences of nursing actions; request supervision when needed; perform within the guidelines established by the Florida Nurse Practice Act. Administrative • Document assessment, plan, and intervention on appropriate Company forms with accuracy, timeliness, legibility, professional terminology and awareness of legal and regulatory implications. • Document patient condition and decline to support hospice and level of care appropriateness. • Present concise and pertinent oral and written reports to IDG; respect and encourage input from all disciplines. • Provide hospital with a copy of the hospice plan of care when patient will remain in the hospital after date of hospice admission. • Schedule staff/provide coverage/assistance to department as needed and as acuity dictates to ensure continuity of care to all patients. • Communicate accurately and completely to physicians, staff members, patients, families, and supervisors; utilize positive approaches when working with others. • Exemplify behaviors supportive of internal mission statement. • Participate in orienting new staff, students, volunteers and board members. • Participate on committees and projects, as assigned. • Act as a resource to peers, students, orientees and other staff members. • Participate in activities and professional development that enhance hospice nursing skills. • Participate in the broad education process of the Company, both to the medical and lay communities. Professional Development • Pursue own professional growth through attendance at required in-services and other educational offerings. • Establish professional goals for self and continually evaluate self in the attainment of these goals. Financial • Utilize supplies, equipment, medications, laboratory and patient therapies within the Company philosophy. • Employ palliative and home care management modalities in care of the patient prior to hospitalization or to promote hospital discharge planning. • Demonstrate knowledge of payment structures and paperwork necessary to change financial classes. Other • Duties as assigned by the Admissions Manager.* Additional RN, Preceptor Responsibilities: Education • Plan, coordinate, and carry out educational opportunities for orientee(s). • Explain the role and performance expectation for the admission nurse in the hospice setting. • Assist the orientee to identify learning needs. • Plan for and provide relevant learning experiences to orientee(s) involving the clinical supervisor, senior preceptor and other staff, necessary. • Orient new nurses to their roles within the department and IDG. • Evaluate competence of orientee utilizing orientation competency checklist. • Provide prompt and consistent feedback, identifying areas in need of further development to the new nurse and the clinical supervisor. • Coordinate activities with other nursing preceptors to maintain consistency in practice, establish relationships, and broaden the scope of the program. • Provide input to clinical leadership and educators regarding patient care experiences for orientation. •Participate in Nurse Preceptor Meetings. |
