Job Description:  United Hospice of Athens is seeking an outstanding RN Case Manager with previouse Hospice experience to join our team
SUMMARY OF DUTIES: Responsible for case management of hospice patients. Must be organized and able to manage time effectively. Case Manager for hospice patients.? Responsible for coordination of care with patient, family & physicians.
Regular hours: Monday • Friday 8:30am • 5pm. Most on-call is covered with PRN nurses, but will occassionally have?back-up call. Territory that is open includes Oglethorpe and Washington counties.? Also, there will probably be some Athens patients.REQUIREMENTSEDUCATION: Current Georgia license as RN.EXPERIENCE: Minimum of 1 year of?nursing experience. 3 years preferred. 1 year of Hospice and/or home health experience A MUST!!!?SKILLS: Excellent oral and written communication skills. Must have good verbal and written skills.
