Job Description:  Provides defined nursing care to a patient in home, long term care facility or assisted living facility under the supervision and direction of a Registered Nurse. Supports and participates in Continuous Care Team. Takes responsibility for communicating timely and thorough care information to the primary caregiver and to the hospice team. Is able to function independently within the parameters of this job description and the Missouri Licensed Nurse Practice Act. Supportive of Village mission and values.
•Provides direct patient care in a safe, quality focused manner.
•Obtains and maintains specific plan of care developed by the RN Case Manager
•Demonstrates competency in patient care tasks
• Participates as a team member supporting the hospice program and philosophy of care
• Promotes and maintains client comfort through teaching caregivers appropriate comfort measures and personal care techniques
• Provides personal care services as directed by the RN Case Manager or Patient Care Supervisor. These services may include:
Reading and recording temperature, pulse and respiration
Assessing for pain
Maintaining a clean and safe environment
Bed, sponge, tub or shower bathing
Shampooing in sink, tub or shower
Caring for nails and skin
Oral hygiene
Assisting with toileting
•Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of clients, transferring clients, assisting with the normal range of motion and positioning and doing household chores
•Prepares and provides adequate nutrition and fluid intake
•Performs homemaking and other environmental services that facilitate receiving sanitary and safe care at home
•Demonstrates utilization of good basic infection control procedures and follows agency procedures, especially with regard to infection control, handling of wastes, and safety measures in all instances.
•Renders service in strict accordance to the plan of care.
•Attends regularly scheduled Interdisciplinary Team meetings and other meetings as required.
•Represents the John Knox Village Hospice program to every constituency and public in a professional manner.
Requirements MO Licensed LPN, preferaly experienced with end of life care.
Valid driver license and proof of auto insurance required.
Intermediate computer usage skills

•Must have the ability to exhibit professionalism, tact and patience when communicating with patients, clients, family’s subordinate staff, staff at all levels and components of the organization, business associates, and the public.
