Position Summary:
Represents the residents’ social interests by planning, organizing, implementing, evaluating and directing the overall operation of the Social Services Department in accordance with current federal, state and local standards (and as may be directed by the Care Center Administrator) to ensure that the medically-related emotional and social needs of the residents are met and maintained on an individual basis. Responsible for the supervision of an Administrative Assistant, where applicable
Key Competencies:
• Responsible for upholding resident rights.
• Provides staff, resident and family education.
• Assists with the admission and/or transfer of residents to the Care Center.
• Participates in quality assessment and performance improvement activities.
• Coordinates discharge planning for the resident and family.
• Excellent problem solving skills.
• Excellent communication and listening skills, enthusiastic, resourceful, organized and possesses strong personal awareness of nonverbal communication.
• Computer skills including proficiency in MS Office.
Education and Experience:
• Education: Completion of four year accredited college in Social Work, Psychology, Gerontology or Sociology.
• Work Experience: Minimum 3 years related experience.
• Licensure / Certification, where required: None
