Job Description: 

The Home Care Coordinator is a registered nurse who oversees the delivery of home care services within the beautiful Vi Lantana CCRC community. This position reports to the Director of Nursing and supervises professional staff, para-professional staff, and Home Health Aides within the community. This agency is not yet Medicare certified, so this position acts as a liaison with contracted Medicare agencies and community personnel. The HCC establishes the plans of care for clients, executes quality improvement programs and provides feedback to QA Nurse, markets SHS to clients and manages to budget, monitors and evaluates identified business metrics and partners with the Assistant Director of Nursing to recommend appropriate staffing levels to meet business demands.

Requirements RN
Bachelor's degree
Must be fluent in healthcare software. SHS uses McKesson software.
Knowledge of Microsoft office.
Strong customer service skills to interact with community personnel, clients, and employees.
Previous experience managing other employees in a healthcare environment.
Political acumen to interact effectively with other management team members in the Vi community.
