The Placement Coordinator provides the link between all internal and external patient placement needs. The Placement Coordinator will have direct contact with patients, families and patient care facilities for the purpose of carrying out the technical aspects of patient placement and transfers. Serves as a liaison with patients, families and patient care facilities. The Coordinator focuses on customer service to patients, families, both internal and external staff and customers, creating a smooth transition from one level of care, or location, to the next. The Placement Coordinator follows all HIPAA requirements with regard to confidentiality of all patient information.
Education: Associate Degree required or equivalent experience in the healthcare setting. LVN/BSW preferred.
Experience: Requires a minimum of 3 years of experience in a health care setting. Experience in Hospice, hospital, home health or SNF/LTC is required, with 1 year of placement/discharge planning experience preferred. Must possess a familiarity with medical terminology. Must have knowledge of Medicare/medical and additional payer sources. Must be able to prioritize and multitask daily duties. Skillful at planning, organizing and completing assignments in a thorough, timely manner. Some program/project development knowledge is preferred. Ability to work cooperatively with multidisciplinary team, including Business Development, to support good customer service collaboration. Must possess strong customer service orientation including interpersonal skills, diplomacy, tact and confidentiality. Proficiency in PC based software programs. Current California Driver’s license and automobile insurance required.
