The Registered Nurse (RN) in our Hospice Homecare program provides skilled nursing care to terminally ill patients in accordance with the treatment and plan of care dictated by patient, family, physician, and members of the interdisciplinary team. The RN works in collaboration with the team Medical Social Worker and Spiritual Counselor (Chaplain) to coordinate care for the patient. Care for the patient is delivered where the patient resides, be it at home, in a skilled facility, hospital, etc., via regionally designed care teams. Employee is expected to learn and utilize a windows compliant computer system for patient charting and intra-departmental communication. Proficiency and appropriate utilization is a requirement of the job role. Available shifts are Full Time, Monday - Friday.
Education: Must be a graduate of an approved school of professional nursing, who is currently licensed as a Registered Nurse by the State of California . A Bachelor's or Associate's degree in nursing from a program accredited by the National League of Nursing is highly preferred. A Public Health Nurse credential and or hospice certification is desirable. Must possess a valid CPR certification. Bilingual abilities preferred .
Experience: Requires a minimum of one year acute care or home health experience as a professional nurse. Good knowledge of home health care principles and practices within the scope of palliative medicine. Familiarity with interpersonal dynamics operative within a health care team and knowledge of the dying and bereavement processes. Possess basic knowledge or aptitude to operate a computer, knowledge of basic software programs is preferred. Current California Driver’s license and automobile insurance required.
