Job Description:  Schedule: Monday-Friday 8am-5pm

General Duties:

1.     Provides consistent quality care for patients with wounds under the direction of the Clinical Manager while communicating and collaborating closely with physicians and other members of the health care team;

2.     Working with a high degree of autonomy, accepts responsibility and accountability for assigned patients as a case manager to plan, implement and evaluate the care for adult and geriatric clients.

 

*The impact of the wound carecase manager position includes the coordination, planning of patient care as well as the delivery of complex nursing interventions, resulting in positive patient outcomes, increased operating efficiencies and improved customer relations/service.

*The RN will assume responsibility for an assigned group of patients and be accountable for an assessment of the patient’s

basic human needs, enabling team of providers to make appropriate decisions to develop, implement and evaluate a plan of care. 

This assessment will include the following factors: biophysical needs, biopsychosocial needs, environmental/safety needs, self

care needs, educational needs and discharge planning needs.

*The RN will provide the patient with care that is continuously evaluated, planned for and revised as needed.  The patient’s status is documented within the reassessments, patient care needs, interventions to meet the patient’s needs, nursing care provided, the patient’s response to, and the outcome of the care provided.  Documentation is clear, concise and includes, but is not limited to patient evaluations, status, care needs, intervention, outcome, and reassessment.

*The RN will organize and prioritize work to provide health information, discharge planning and patient care in a safe environment to maximize health in the continuum of care and protect the patient.

 

Requirements:

Two (2) years of experience in acute care nursing, experience in ambulatory care setting highly desirable.

Wound Care experience preferred

Case Manager/Discharge Planning experience preferred
