The Quality Manager is responsible for administering quality initiatives within the Home Care Division of CHS. The primary responsibilities include assisting the agencies with CHAP accreditation including mock surveys, readiness initiates and performance improvement plans when needed. The position is also responsible for patient surveys and analysis of survey data. The Quality Manager will also assist in the development and presentation of quality improvement training programs.





The Quality Manager must be a graduate of an accredited school of nursing. A BSN, MSN or masters in related field is preferred. Two (2) or more years of experience in home health or hospice is required.


Must be knowledgeable and experienced with Microsoft Word, Excel, PowerPoint and Lotus Notes. Must be able to travel up to 50% of the time.
