This position is responsible for the development and implementation of administrative and clinical programs that will assure that hospice patients receive safe, appropriate and cost-effective drug therapy.

This clinical pharmacist position works collaboratively with other members of the interdisciplinary team to ensure that hospice patients receive safe, appropriate and cost-effective drug therapy. In addition, this position is responsible for reviewing and interpreting drug regimen reviews, developing and implementing clinical programs, providing continuing education for clinical staff and Pharmacy students, and directing the work of support staff to maximize positive patient outcomes.
Requirements:.
Doctor of Pharmacy degree with clinical experience. A valid Florida license to practice pharmacy and an active license of consultant pharmacist. Minimum of five years experience in acute or ambulatory patient care and/or advanced pharmacy residency training. Robust experience in reviewing and interpreting drug regimen reviews and/or medication therapy management. Experience in managed care and training and/or experience in the palliative care of hospice patients and pain management are an added value. Demonstrates exceptional skills in collaboration, teamwork, problem-solving, and communication.
