POSITION SUMMARY


 
The Director of Nursing directs, supervises and coordinates all clinical services provided to home care patients within the assigned geographical areas.


ORGANIZATIONAL STRUCTURE

 

The Director of Nursing is accountable to the Administrator.


  RESPONSIBILITIES


  1.      Assures that day to day operations run smoothly and efficiently within established guidelines.


2.      Assures that Agency policies and procedures are enforced and followed by staff.


3.      Admits new patients in the geographical area serviced by the team and completes the SOC and 485 paperwork, including OASIS as needed.


4.      Supervises the clinical management of patients assigned to the team.


5.      Supervises staff assigned to the team, including evaluating performance and competency skills, disciplinary counseling, recommending promotion and/or termination, as indicated.


6.      Conducts supervisory visits on staff members assigned to the team to assure skills/care is appropriate.


7.      Assures clinical staff submits clinical notes and other required paperwork timely per Agency policy.


8.      Conducts reassessment visits and completes recertification paperwork, including OASIS.


9.      Conducts post-hospital assessment visits and completes the paperwork, including OASIS.


10. Completes prebill audits for patients on team to assure accurate and timely submission of claims.


11. Randomly reviews clinical notes completed by team members to assure documentation is appropriate to patient care and the physician’s plan of care.


12. Takes initial referral information on patients referred to the Agency.


13. Serves as case manager for all cases assigned to the team.


14. Evaluates outcomes of patient care and consults with other disciplines, and adjusts level of care as necessary to assure optimal patient care.


15. Oversees the delivery of patient care, directing and delegating patient care assignments based on the needs of the patient.


16. Demonstrates consistent and fair management skills without favoritism.


17. Assures utilization is appropriate to patient needs.


18. Demonstrates ability to organize, prioritize, motivate, and delegate appropriately.


19. Assists the Administrator and Director of Performance Improvement in developing and revising policies and procedures, as necessary.

 

20. Serves as a role model to staff by performing own job responsibilities professionally, timely, efficiently, and correctly.


21. Provides appropriate orientation, training, and education to staff.


22. Performs other duties as assigned.


 


POSITION QUALIFICATIONS


 


1.      Minimum education requirements: RN from an accredited School of Nursing .


2.      Minimum experience requirements: 5 years experience as a RN; 2 years of home care nursing, and 1 year of supervisory experience.


3.      Knowledge of Medicare, TennCare, state and federal regulations, and accreditation standards as they relate to home care.


4.      Current Tennessee Registered Nurse license.


5.      BLS Certification.


6.      Valid driver’s license and proof of insurance.
