| Job Description: | Will conduct Home Health Physical Therapy visits as needed. Will be responsible for evaluation and treatment of disabilities, injuries, and diseases in accordance with the orders of the referring physician. Performs assessments and establishes a plan of care. Maintains documentation for each patient in compliance with Agency policies and HCFA regulations Requirements Must be a Registered Physical Therapist in Kansas and/or Missouri (dual licensed within 60 days of employment.) Bachelor of Science or higher in Physical Therapy required. Must have current driver license and proof of auto insurance. |
