The Social Worker will implement, develop, and evaluate patient care and social services interventions in accordance with title xxii and the Agency policies and procedures. Assists the interdisciplinary team in meeting psychosocial needs of patients to enable patient to utilize medical and nursing care.


Performs other related duties as assigned. Position purpose statements are intended to describe the general nature of work to be performed and are not intended to be construed as an exhaustive list of all responsibilities. Management reserves the right to reassign duties as needed.


The BSW performs duties in accordance with and under direction/supervision as defined by the agency's organizational chart. This position works out of both our Fulton and Mayfield agencies to serve patients throughout the eight counties of southwest Kentucky.





Education:



  • Minimum: Bachelor's Degree in Social Work.



Experience:


  • Minimum of one year in Health Care. Able to communicate well verbally and in writing, problem solve, and manage crisis situations.

  • Must adhere to National Association of Social Workers Code of Ethics


Licenses/Certifications:




  • MUST BE CERTIFIED / LICENSED IN KENTUCKY BY AMERICAN SOCIAL WORKERS' BOARD.





  • Valid driver's license and automotive insurance with reliable transportation and DMV verification of driving record. Current CPR card.
