Job Description: 

Position Requirements: Current North Carolina license as an RN and current CPR Certification. A minimum of three (3) years discipline specific experience, preferably with Hospice or home care. General understanding of disease process and ability to explain Hospice services and reimbursement issues. Demonstrates good nursing assessment and critical thinking skills. Demonstrates excellent communication skills, both oral and written. Ability to work well within an interdisciplinary team setting as well as independently with a minimum of supervision. Commitment to and understanding of the Hospice concept and thorough understanding of agency policies and procedures. Excellent organizational skills with the ability to handle a variety of tasks and to assign priorities to those tasks. Ability to work a flexible schedule.

Position Duties and Responsibilities: Supervise the day-to-day activities of the interdisciplinary team including coordination of visits to ensure quality patient care. Provide an ongoing assessment of the clinical competencies of the clinical staff as needed. Provides coaching, mentoring, and counseling to staff as needed. Provide field observation and training to ensure compliance with Nurse Practice Act, North Carolina Board of Nursing, and all applicable regulatory agency standards. Complete/Supervise completion of clinical documentation according to agency, state, federal and accreditation standards. Collect and analyze data in an effort to implement, coordinate, maintain and evaluate all levels of patient care and services provided. Work with Director, VP, and Human Resources to document and resolve disciplinary issues. Timely completion of employee evaluations. Interviewing, hiring, orienting, and training new staff. Coordinate/complete the admissions and delivery of Hospice & Palliative Care services to patients living in personal residence and long term care facilities on an as needed basis. Establish and foster working relationships with patients, family members, professional and paraprofessional agency staff members and personnel of other agencies. Coordinate RN Case Management assignments in a manner that ensures cost effectiveness related to mileage reimbursement. Plan and maintain staffing for the team based on productivity benchmarks, as well as, annual, monthly and daily volume trends. Lead and coordinate Team/Care Planning meetings and have a direct involvement in developing, implementing, and evaluating plan of care. Participate in preparing staff for surveys actively involving them in quality improvement, monitoring, and reporting. Participate in long range strategic planning for department and agency, including program development and expansion. Work closely with other Team Leaders to be a resource to all disciplines to ensure continuity of quality care. Ensure customer service expectations of patients, families, physicians, long term care facilities, and colleagues are met or exceeded. Attend agency-wide managers meetings and clinical manager meetings as scheduled. Provide nursing services on an as needed basis according to the plan of care that has been established and in accordance with the Nurse Practice Act and agency standards of care. Triage patient/MD/referral calls. Assist in providing 24-hour On-call services 7 days a week. Perform other duties as assigned by supervisor.
