Job Description:  About the Opportunity Brentwood Sub Acute Health Care Center a 163 bed facility skilled nursing facility located in Burbank Illinois. Brentwood Sub Acute Health Care Center is located at 5400 West 87th St Burbank Illinois 60459. The staff physical therapist screens, evaluates and treats patients/residents; communicates with families, physicians and other health team members; and maintains documentation of services in the medical record. The PT also conducts inservices and training for faculty and staff on an ongoing basis in restorative techniques and supervises PTA's and Aides as required. Qualifications Qualified candidates must have graduated from an accredited program and possess (or be eligible for) a current PT license in the State where the facility is located. New Grads are welcome!
