TITLE: Medical Social Worker

REPORTS TO: Clinical Coordinator

POSITION SUMMARY:

The Social Worker assists patients and families with personal and environmental difficulties that interfere with obtaining maximum benefits from care.

QUALIFICATIONS:

1. Masters of Social Work accredited by the Council of Social Work Education preferred and as required per state standards.
a. Bachelors of Social Work accredited by the Council of Social Work Education is acceptable for hospice services if applicable to state standards. BSW employed prior to 12/2/2008 are not required to be supervised by a MSW.
2. Licensed as a Social Worker if required in the state of practice.
3. One year of social work experience in a health care setting.
4. Hospice or home health experience preferred.

RESPONSIBILITIES AND DUTIES:

1. Assists the physician and other members of the health care team in understanding significant social/mental stress and/or disorder that exacerbates the symptoms related to terminal illness.
2. Assesses the patient's social and emotional status in order to estimate the client's capacity and potential to cope with problems of daily living and assesses the caregiver psychosocial status.
3. Participates in development and revision of plan of care to assist the patient in resolving difficulties, which affect his/her ability to benefit from care provided.
4. Helps patient and family to understand, accept and follow medical recommendations.
5. Assesses the patient/family psychosocial status, potential risk of suicide and/or potential for abuse or neglect.
6. Assesses emotional factors related to terminal illness.
7. Assesses environmental resources and obstacles to maintain safety.
8. Provides social services including short-term individual counseling, crisis intervention, assistance in providing information and preparation of advance directives, funeral planning and issues involving transfer of fiscal, legal and health care responsibility.
9. Identifies family dynamics, communication patterns and involves the family/caregiver in the plan of care.
10. Identifies and utilizes appropriate community resources and assesses patient/family/caregiver's ability to access them.
11. Evaluates patient/family response to psychosocial interventions.
12. Assesses caregiver's ability to function adequately.
13. Assesses need for counseling related to risk assessment for pathological grief.
14. Assesses special needs related to cultural diversity including communication, space, role of family members and special traditions.
15. Identifies the development level of patient/family/caregiver and obstacles to learning or ability to participate in care of patient.
16. Addresses patient/family questions and issues.
17. Identifies obstacles to compliance and assists in understanding goals of intervention.
18. Identifies support systems available to reduce stress and facilitate coping with end-of-life care.
19. Communicates psychosocial information to inpatient facility when level of care is changed.
20. Assists family is assessing financial resources when appropriate.
21. Attends team meetings, and maintains effective communication with other team members.
22. Identifies patient/family needs when discharged or when level of care changes.
23. Evaluates patient/family response to intervention when referred to community agency and satisfaction of service provided.
24. Prepares clinical and progress notes.
25. Assesses bereavement needs.
26. Participates in in-service programs.
27. Evaluates the patient for long-term care when appropriate and assesses ability to accept change in level of care.
28. Is involved in the QAPI Program.
29. Performs other related tasks as requested.

PHYSICAL REQUIREMENTS:

Strength:
Frequently required to push/pull and lift up to 25 pounds of weight.

Manual Dexterity:
Constantly required to perform simple motor skills and manipulative skills such as standing, walking, writing and use of various types of therapeutic and office equipment. Frequently required to perform moderately difficult manipulative skills such as positioning patients and use of more complex office and therapeutic equipment.

Coordination:
Constantly required to perform gross body coordination such as walking, stooping, standing while performing tasks, carrying and lifting objects.

Mobility:
Frequently required to walk and stand, including for long periods of time. Frequently required to remain in uncomfortable positions for long periods of time, such as bending over equipment, counter, tables, etc.

Visual Discrimination:
Constantly required to see objects closely to allow reading instructions, physician orders, computer screen and measurement tools. Frequently required to discriminate colors on charts, equipment panels or warning lights. Occasionally required to use depth perception and judge distance, size, color and shape of an object.


MENTAL REQUIREMENTS:

Constantly required to concentrate on fine details with continual interruptions; attends to a task/function for more than 60 minutes at a time; understand and relate to specific ideas; remember tasks/assignments given at the beginning of a period of time extending several days. Constantly required to demonstrate good judgment and safety.

POST-OFFER HEALTH STATUS:

Meets the post-offer physical requirement regarding infectious/contagious disease, including TB screening, and other regulatory requirements.

ENVIRONMENTAL REQUIREMENTS:

Potential exposure to occupational hazards which may include, but are not limited to infectious/ contagious diseases, heat, cold, noise, hazardous substances, uncontrolled home environments, varying weather conditions and other hazards which may be located at various work sites.

TRAVEL REQUIREMENTS:

Required to drive a car long distances from one location to another.

OSHA CATEGORY FOR BLOODBORNE PATHOGENS EXPOSURE:

Category II: Tasks and procedures involve potential occupational exposure to blood, body fluids and/or tissues.
