Fairview hospital is currently recruiting an Adult, Family, or Acute Care Nurse Practitioner or PA to join our team. The successful applicant will be serving a population of pre-surgical patients in collaboration with the Anesthesia Institute to prepare patients for surgery and put them at the best possible level of health for their procedure. The NP/PA will perform a thorough history and physical and will diagnose, manage, and treat the patient's medical conditions appropriately in collaboration with the anesthesia and in accordance with the ONE PACC guidelines in preparation for their procedure. The goal will be for the NP/PA to optimize the patient for surgery by ordering treatments/consults required by their medical conditions. Prefer an experienced adult, family, or acute care NP or PA. Prefer backgrounds in surgery, critical care, or internal medicine. Functions in collaboration with physicians according to the Standard Care Arrangement or Supervisory Agreement and specialty certification. NP's must have prescriptive authority in the One PACC model.

The new Pre-Anesthesia Consultation Clinic (PACC) will be located at 850 Columbia Road in Westlake, Ohio.

Summary: Nationally certified licensed healthcare provider who provides primary and/or specialty nursing and/or medical care in a variety of healthcare settings. Functions in collaboration with a physician according to a standard care arrangement and specialty certification. Practice emphasizes health promotion, disease prevention and the diagnosis and treatment of acute and chronic health problems. Certifications may include: acute care, adult, family, gerontology, psych and mental health, neonatology,pediatrics and women's health. Job Responsibilities: Assess health status by obtaining relevant health and medical history. Performs a physical exam based upon the age and patient history as per the Standard Care Arrangement with a collaborating physician. Identifies significant health and medical risk factors. Orders, interprets and evaluates laboratory test and other diagnostic studies to assess patient's clinical problems and health care needs. Develops a diagnosis by analyzing and synthesizing collected data. In collaboration with the physician, develops a treatment plan along with the patient and significant others. Performs diagnostic or therapeutic interventions consistent with certification, law and Cleveland Clinic privileges. May prescribe and monitor medications as allowed by prescriptive authority through applicable state Board of Nursing and CCF policy. Develops patient education plan. Participates in medical team rounds; collaborates with nursing, medical and other healthcare team members regularly to ensure quality patient care. May perform admission history & physical, document progress notes, prepare discharge summaries as required. Assists with and facilitates discharge planning. Other duties as assigned. Education: Master's or Doctoral Degree in Nursing. Experience: Two years clinical nursing experience preferred. Licensure/Certification/Registration: Currently Licensed as RN in applicable state, national certification as nurse practitioner by nationally accredited organization, current certificate of authority by applicable state Board of Nursing, certificate to prescribe preferred. BLS Healthcare Provider required; ACLS or PALS may be required based upon work setting. Physical Requirements: Typical physical demands include the ability to walk and stand for long periods of time. Manual and finger dexterity and eye/hand coordination to perform physical examinations. Requires corrected vision and hearing to a normal range. Occasionally lifts and carries items weighing up to 50 pounds. May require exposure to communicable diseases and/or body fluids.
